Headache on GLP-1 Medications: What to Know

Headaches are a commonly reported side effect of GLP-1 medications, particularly during the initial weeks of treatment. They are often linked to changes in blood sugar levels, dehydration from reduced food and fluid intake, and the body's overall adjustment to the medication. Most headaches are mild to moderate and respond well to standard remedies.

How Common Is It?

Reported rates of headache vary by medication:

Rates shown are adult incidences from the clinical-trial sections of each medication's FDA prescribing information, checked on 31 July 2026. Pediatric trial rates differ — Wegovy vomiting is 24% in adults and 36% in patients aged 12 and older. Where a label reports a symptom only through post-approval reports, FDA states its frequency cannot be reliably estimated, so we show that rather than a number. Where a label does not list the symptom at all, we say so — that means no label basis for a rate, not that the symptom is rare or impossible. A symptom reported during treatment is not by itself proof the medication caused it.

Why Do GLP-1 Medications Cause Headache?

  • Blood sugar fluctuations during the adjustment period can trigger headaches, especially if meals are skipped
  • Dehydration from reduced fluid and food intake is a major contributor to headaches on GLP-1 medications
  • GLP-1 medications can affect blood pressure regulation during the adjustment period, which may contribute to headaches
  • Reduced caffeine intake due to appetite changes can cause withdrawal headaches in regular coffee or tea drinkers

When Does It Happen?

Headaches on GLP-1 medications are most common during the first 2-4 weeks and after dose increases. They typically occur 1-3 days after injection when medication levels are highest. Most patients see headache frequency decrease significantly after the first month of treatment.

How to Manage Headache on GLP-1 Medications

1

Drink plenty of water throughout the day, aiming for at least 64 ounces to combat dehydration

2

Maintain regular meal times and avoid skipping meals, even if appetite is reduced

3

Over-the-counter pain relief such as acetaminophen or ibuprofen can help manage acute headaches

4

Monitor blood sugar levels if you have diabetes, as hypoglycemia can trigger headaches

5

Reduce caffeine intake gradually rather than stopping suddenly to avoid withdrawal headaches
